PT-141

Composition

Each pack contains:

• One vial with PT-141 (Bremelanotide) – 10 mg
• One ampoule with 2 ml sterile water for injection

Pharmacological Effects

• Melanocortin receptor agonist: Synthetic analog derived from Melanotan II, primarily targeting MC4R and MC1R.
• Sexual dysfunction treatment: Stimulates central arousal pathways (acts on the brain, unlike PDE5 inhibitors which improve blood flow).
• Mood modulation: Some users report mild anxiolytic effects.
• Minimal tanning: Much weaker MC1R activation than MT-2.

Indications

• FDA-approved (2019): Hypoactive Sexual Desire Disorder (HSDD) in premenopausal women.
• Off-label/Emerging Uses:
  • Erectile dysfunction (especially PDE5 non-responders).
  • Female sexual arousal disorder.

Recommended Dose

FDA-Approved (Vyleesi)

• 1.75 mg subcutaneous injection (auto-injector), taken 45 minutes before sexual activity.
• Max frequency: Once every 24 hours; do not exceed 8 doses per month.

Compounded / Research Use (10 mg/vial – Off-label)

• Reconstitution: Typically with 1 ml sterile water → 10 mg/ml concentration.
• Starting dose: 0.5–1 mg SC (to assess tolerance).
• Therapeutic range: 1–2 mg SC, 30–60 minutes before activity.
• Max dose: Do not exceed 2 mg due to increased side-effect risk.

Side Effects

Common

• Nausea (~40%).
• Flushing.
• Headache.
• Mild hypertension (blood pressure increase).
• Yawning, dizziness, or vomiting (less common).

Serious (Rare)

• Hypertensive crisis (avoid in uncontrolled hypertension).
• Priapism (erection >4 hours; emergency condition).
• Darkening of gums or skin patches (MC1R-related).

Contraindication

• Uncontrolled hypertension.
• Cardiovascular disease (risk of severe BP elevation).
• Pregnancy (Category C – unknown fetal safety).
• Concurrent use with nitrates or PDE5 inhibitors (additive BP-lowering interactions).

Storage Conditions

• Unreconstituted: Store at 2–8°C (36–46°F); stable for up to 24 months.
• Reconstituted: Stable for 7 days refrigerated (with bacteriostatic water).
• Do not freeze once reconstituted.
• Protect from light to prevent degradation.
