Description
Composition
Each tablet contains:
- Tadalafil – 20 mg
- Excipients – q.s.
Pharmacological Effects
Phosphodiesterase-5 (PDE5) Inhibitor
- Increases nitric oxide (NO) effects, leading to vasodilation in corpus cavernosum.
- Causes smooth muscle relaxation, enhancing blood flow for erectile dysfunction (ED).
- Long half-life (~17.5 hrs) allows up to 36-hour duration of action (“weekend pill”).
- Mild PDE11 inhibition (unknown clinical significance).
Indications
- Erectile Dysfunction (ED): 10–20 mg before sexual activity.
- Benign Prostatic Hyperplasia (BPH): 5 mg once daily.
- Pulmonary Arterial Hypertension (PAH): 40 mg once daily (Adcirca®).
Recommended Dose
1. For Erectile Dysfunction (ED)
On-Demand Use:
- Standard dose: 10–20 mg taken 30–60 minutes before sexual activity.
- Maximum frequency: Once per day.
- Duration of effect: Up to 36 hours (often called the “weekend pill”).
Daily Low-Dose Use (Chronic ED):
- 2.5–5 mg once daily (taken at the same time each day).
- Preferred for men who anticipate frequent sexual activity.
2. For Benign Prostatic Hyperplasia (BPH)
- 5 mg once daily (can be taken at any time of day).
- May be combined with tamsulosin (alpha-blocker) in some cases.
3. For Pulmonary Arterial Hypertension (PAH)
- 40 mg once daily (as Adcirca®, a specialized formulation).
Side Effects
Common:
- Headache
- Flushing
- Dyspepsia
- Nasal congestion
- Back pain
- Myalgia
Serious (Rare):
- Priapism (prolonged erection >4 hrs – medical emergency).
- Sudden hearing or vision loss (NAION – non-arteritic ischemic optic neuropathy).
- Hypotension (use caution with nitrates).
- Cardiovascular events (avoid in unstable angina or heart failure).
Contraindications
- Concomitant nitrates (e.g., nitroglycerin) – risk of severe hypotension.
- Hypersensitivity to tadalafil or other PDE5 inhibitors.
- Severe cardiovascular disease (recent myocardial infarction or stroke).
- Retinitis pigmentosa (risk of vision impairment).
Storage Conditions
- Store below 30°C in original packaging to protect from moisture.
- Keep out of reach of children.
