Composition

• One vial with Recombinant Human Growth Hormone (Somatropin) – 10 IU
• One ampoule with 2 ml sterile water for injection

Pharmacological Effects

• Direct GH replacement: Binds to GH receptors in liver and muscle, stimulating IGF-1 production.
• Anabolic effects: Increases protein synthesis, muscle growth, and promotes lipolysis (fat breakdown).
• Metabolic effects: Enhances glucose uptake but may have anti-insulin behavior at higher doses.
• Tissue repair: Supports collagen synthesis, bone/cartilage growth, and general tissue regeneration.

Indications

• Growth Hormone Deficiency (GHD) – pediatric and adult.
• Turner syndrome (in girls).
• Chronic Kidney Disease (CKD) — growth failure in children.
• Prader–Willi Syndrome (with growth failure).
• Short Bowel Syndrome (adjunct therapy).
• HIV-associated wasting (approved in some regions).

Recommended Dose

Adults (GHD)

• Starting dose: 0.15–0.3 mg/day (0.45–0.9 IU/day) SC or IM.
• Maximum dose: 1–2 mg/day (3–6 IU/day), depending on condition and tolerance.

Pediatrics

• GHD: 0.025–0.035 mg/kg/day (0.075–0.105 IU/kg/day).
• Turner Syndrome: 0.045–0.05 mg/kg/day (0.135–0.15 IU/kg/day).

Side Effects

• Injection site reactions (pain, redness, swelling).
• Arthralgia/myalgia (joint/muscle pain).
• Edema due to fluid retention.
• Headache and occasional numbness/paresthesia.

Contraindication

• Active malignancy (GH-induced IGF-1 may stimulate tumor growth).
• Diabetic retinopathy (risk of progression).
• Acute critical illness (e.g., post-surgery, traumatic injuries, respiratory failure).
• Hypersensitivity to somatropin or formulation excipients.

Storage Conditions

• Unreconstituted vials: Store at 2–8°C (refrigerated). Do NOT freeze.
• Reconstituted solution:
  • Sterile water: Use immediately; discard unused portion.
  • Bacteriostatic water: Stable for 14–28 days at 2–8°C (refer to manufacturer guidelines).
• Protect from light and avoid excessive shaking/agitation.
